LIABILITY RELEASE AND WAIVER

I desire to participate in Camp Neshama  to be held at American Jewish University-Brandeis Bardin Campus, and I fully understand and appreciate the dangers, hazards, and risks inherent in all activities. Knowing the dangers, hazards, and risks of such activities, and in consideration of being permitted to participate in Camp Neshama, on behalf of myself, my family, heirs, and personal representative(s), I agree to assume all the risks and responsibilities surrounding my participation in Camp Neshama, and in advance release, waive, forever discharge, and covenant not to sue JConnect Inc., Pico Shul, Beth El Synagogue, or American Jewish University-Brandeis Bardin Campus, its governing board, officers, agents, and employees, from and against any and all liability for any harm, injury, damage, claims, demands, actions, causes of action, costs, and expenses of any nature that I may have or that may hereafter accrue to me, arising out of or related to any loss, damage, or injury, including but not limited to suffering and death, that may be sustained by me or by any property belonging to me, whether caused by the negligence or carelessness of the staff, or otherwise, or any adjunct to Camp Neshama.

I understand and accept that Camp Neshama will not have medical personnel available during the weekend. I understand and agree that Camp Neshama  staff are granted permission to authorize emergency medical treatment, if necessary, and that such action by staff shall be subject to the terms of this Agreement. I understand and accept that the American Jewish University-Brandeis Bardin Campus, Pico Shul, JConnect or Beth El Synagogue assume no responsibility for any injury or damage which might arise out of or in connection with such authorized emergency medical treatment.

It is my express intent that this release and hold harmless agreement shall bind the members of my family and spouse, if I am alive, and my estate, family, heirs, administrators, personal representatives, or assigns, if I am deceased, and shall be deemed as a Release, Waiver, Discharge, and Covenant not to sue the above-named American Jewish University-Brandeis Bardin Campus, Pico Shul, JConnect Inc, Beth El Synagogue. I further accept to save and hold harmless, indemnify, and defend American Jewish University-Brandeis Bardin Campus, Pico Shul, JConnect and Beth El Synagogue  from any claim by me or my family, arising out of my participation in Camp Neshama.

In accepting to this Release, I acknowledge and represent that I have fully informed myself of the content of the foregoing waiver of liability and hold harmless agreement by reading it before I accept it, and I understand that I agree to this document as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written statement, have been made. I understand that American Jewish University-Brandeis Bardin Campus, Pico Shul, JConnect Inc. and Beth El Synagogue do not require me to participate in this activity, but I want to do so, despite the possible dangers and risks and despite this Release. I further state that I am at least eighteen (18) years of age and fully competent to accept this Agreement and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my participation in this activity and that I have adequate health insurance necessary to provide for and pay any medical costs that may be attendant as a result of sickness or injury to me and that Camp Neshama  shall have no responsibility for the payment of same. I agree that my acceptance denotes my affirmation that I will abide by all state, local and federal laws.

PHOTO RELEASE
I hereby grant JConnect Inc., Beth El Synagogue and Pico Shul permission to use my likeness in a photograph or other digital reproduction in any and all of its publications, including website entries, without payment or any other consideration.

I understand and agree that these materials will become the property of JConnect Inc., Beth El Synagogue and Pico Shul and will not be returned. I hereby irrevocably authorize JConnect Inc, Beth El Synagogue and Pico Shul to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing the its programs or for any other lawful purpose.

In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge JConnect Inc., Beth El Synagogue and Pico Shul from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am 18 years of age and am competent to contract in my own name. I have read this release before accepting it and I fully understand the contents, meaning, and impact of this release.
